A tongue tie is a significant reason for maternal nipple discomfort and trauma. It impedes breast milk transfer and in many cases leads to untimely breastfeeding cessation.

A tongue tie is a short thick tight band of tissue that tethers the bottom of the tongue to the floor of the mouth, inhibiting the full range of motion. Both Anterior and Posterior tongue ties may make breastfeeding very difficult. 

Many new families report a range of potential symptoms that may include
1. Nipple pain at latch-on and sometimes throughout the nursing session.
2. Nipple trauma such as blisters, cracks, and creases on nipples after nursing
3. Leaking milk out the side of the mouth
4. Clicking sounds while nursing indicating ineffective milk removal
5. Frequent plugged ducts

This leads to 
1. Low or slow weight gain
2. Clamping the nipples rather than rhythmic sucking
3. Inability to lift the tongue from the base of the mouth, the tongue looks heart-shaped when the baby cries
4. Gassiness, colic or reflux symptoms

What's helpful is using C Hold to get much breast tissue in the baby’s mouth. Laid back position which can use gravity to help the baby achieve a deeper latch

Furthermore, the Tongue Tie is corrected surgically. A pediatric dentist and a skilled lactation consultant can help the family and ensure no other possible breastfeeding issues. 

I'm truly grateful to support a new mother with a 1-month baby with an anterior Tongue Tie. I am delighted that the mother-baby dyad is enjoying good health and yielding adequate milk removal.

When a new family wanted to be prepared with Antenatal Milk Expression (AME), support them with knowledge, empathy and with a successful and achievable plan of care 

It is important to remember, that not every expecting mom will be able to collect colostrum - but learning the skill of hand expression is certainly valuable before birth and after birth. 

Human milk production begins around the 16th week of pregnancy.

Antenatal milk expression is when a woman uses hand expression to express colostrum several times a day often starting at around 36 weeks’ gestation. ( Done with the permission of the HCP) 

This colostrum is saved, stored, and safely used after the birth of the baby. 

ABME can be given to the babies with Jaundice & Babies in NICU after birth offering the best nutrition.

Antenatal Breast Milk Expression (ABME) may be a helpful tool in improving maternal breastfeeding confidence and breastfeeding outcomes, rerouting the postpartum depressions in early Breastfeeding journey.

But they also have disengagement cues, signaling when they need a change. Whether it's turning away, crying, or showing signs of fatigue, they communicate their needs clearly.

For newborns, feeding cues are particularly important. From clenched fingers to sucking motions, they tell us when they're hungry and ready to feed.

Neglecting these cues can impede the baby's growth and lead to less joyful breastfeeding experiences. Let's tune in and honor these non-verbal signals for healthier, happier babies
